The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient-centered hospice that maximizes resources at bedside while being operated locally. Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve. When end-of-life care is necessary, patients and their families are in their most vulnerable time. Our Ohio-based management team, through our local clinicians, can respond to the needs of our patients in a timely and appropriate manner because we are local. We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Dayton, OH area. We don’t require high caseloads or unmanageable quotas, which gives our team more time with each of their patients. Our top priority is our patients and the people serving them. Schedule: 3 12-hour shifts per week, including every other weekend. What We offer: Great culture and team atmosphere 401(k) retirement plan with a generous company match Mileage reimbursement Employee Referral Program Hospice Certification benefit and salary increase Merit Increases Employee Discount Programs What You’ll Do: Provide direct patient care as defined in State Nurse Practice Act Implement current nursing practice following a comprehensive assessment and the plan of care Provide accurate and timely documentation of patient services to reflect the plan of care Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications Qualifications: Graduate of an accredited practical nurse or a vocational nursing program Three years of nursing experience.
